Sometimes the Puppet Tool assumes the boundaries of your image are smaller than they are (perhaps just by a few pixels). They key to avoiding this is to use the 'Expansion' option. So, select your layer's Puppet Tool effect, then the actual Puppet Tool.
On the top of the screen is that Expansion setting. Try popping that up to a slightly higher number; that should fix your issue. The issue may still occur if your layer is an animate precomp that changes sizes/shape. The Puppet Tool only generates its distortion mesh based on the frame you initially apply it on, so if something in that precomp changes and it's bigger/longer/taller/etc, you may experience a break. Oh, one possible issue with the Expansion setting you might encounter: Your image's fingers. If your puppeting the individual fingers, and you crank the expansion setting too high, you may not be able to move them individually (or not easily). That's because the image's mesh can be expanded so much that the plugin doesn't think there's any space in between the fingers. The only way to work around that is to crank the expansion down once again. It might take a while to dial it into a setting that works perfectly. I saw this post yesterday and actually just had the same thing happen.
I figured out why my problem was so i thought i'd explain if anyone else is reading this post and has the same issue. Like said, expansion will work as a quick fix. My problem was that i had puppet layers (arms) parented to other layers (body) and i keyframed the body layers position. This caused the arm to move with the body (what i wanted) but the puppet mesh stayed stationary.

This means all the layers are the 'size' of your comp (anchor point in center) but like i said i moved the anchor points anyway. The puppet tool i guess 'reads' your layer size (and since its the size of the whole comp instead of just the arm, there is no boundary for it to cut off). I thought that doing it this way the puppet tool would then create mesh for each layer that was the size of the entire comp, but for some reason it didnt.
